Job Title: Case Manager
Job Summary: We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing comprehensive case management services to individuals experiencing homelessness. You will work closely with program participants, helping them navigate through various challenges and barriers, and empowering them to achieve stability and independence. The ideal candidate will have strong interpersonal and organizational skills, a solid understanding of social services, and a passion for making a positive impact in the lives of others.
Responsibilities:
- Conduct comprehensive assessments of program participants' needs, strengths, and goals.
- Develop individualized service plans in collaboration with program participants, focusing on housing stability, income, healthcare, and overall well-being.
- Provide ongoing case management support, including regular check-ins, goal setting, and progress monitoring.
- Assist program participants in accessing and coordinating appropriate services, such as mental health resources, healthcare providers, employment agencies, and housing providers.
- Advocate for program participants' rights and needs, ensuring they have access to necessary resources and services.
- Provide crisis intervention and support to program participants during challenging situations.
- Maintain accurate and up-to-date case records, progress notes, and documentation in compliance with agency standards and confidentiality guidelines.
- Collaborate with internal team members, community partners, and external agencies to coordinate services and support program participants' overall care.
- Participate in regular team meetings, case conferences, and professional development activities.
- Stay current on relevant social service programs, resources, and best practices to enhance service delivery.
- Adhere to agency policies, procedures, and ethical guidelines.
Requirements:
- Bachelor's degree in Social Work, Psychology, or a related field.
- Proven experience in case management or a related field, preferably working with individuals experiencing homelessness or vulnerable populations.
- Knowledge of social services, community resources, and government assistance programs.
- Strong assessment and problem-solving skills to identify individual needs and develop appropriate service plans.
- Excellent communication and interpersonal skills to establish rapport and build trusting relationships with program participants.
- Ability to work collaboratively with a diverse team and community partners.
- Knowledge of crisis intervention techniques and the ability to respond effectively in high-pressure situations.
- Proficient in documentation and record-keeping, with attention to detail and confidentiality.
- Ability to work independently, prioritize tasks, and meet deadlines.
- Understanding of trauma-informed care and cultural sensitivity.
- Valid driver's license and reliable transportation (if required for the position).
